NDIS REFERRALS

Make an NDIS provider enquiry

A short summary is enough to start

What to include

Provide your name, contact details, general location and the type of support you are seeking. You can describe preferred days, communication needs, languages and worker preferences. Tell us whether the enquiry is for you or someone you support. Plan management type may be requested if it is needed to identify a relevant provider; you can choose unsure and discuss it later.

We use these details to manage your enquiry and seek permission for an introduction. Please do not upload a full plan, diagnostic report, identity document or medication record. Before sharing your information, we will identify the receiving provider, the purpose  and the details proposed for transfer.

Communication and contact preferences

Tell us whether you prefer a telephone call, email or contact through an authorised supporter. You can include language and accessibility preferences and any times when it is unsuitable to contact you. If a particular provider should not receive your information, say so before the introduction. You can ask to see the proposed recipient and the information to be sent.

When someone helps with the enquiry

If you are enquiring for another person, explain your relationship and the authority relevant to this request. Involve the person in describing their preferences wherever possible. We may need clarification before information is shared. Being a family member or helping with daily support does not automatically establish authority for every decision.

What happens after submission

Submitting the form records a request for contact. It does not book a worker, establish a service agreement or approve a claim. The receiving provider must confirm whether it can assist and explain any further information it needs. You can decline the next step or ask for another option where one is available.
