NDIS REFERRALS
Home and living enquiries
Consider your support arrangements and continuity
Explore a home and living conversation
Home and living enquiries may involve different support models and accommodation arrangements. Tell us whether you want to discuss support in your current home or a possible move. Give a general location preference and describe the practical assistance you want to explore. We can seek a provider contact where a relevant business is available, without promising a vacancy or a funded arrangement.
The provider must explain the distinction between support services, housing costs, tenancy arrangements and any specialised accommodation offering. Ask who owns or manages the property, who delivers support and whether you can change providers. Any funding approval and suitability assessment must be addressed through the appropriate process. A referral is not an offer of housing, an approval for continuous support or a guarantee that rent and ordinary living costs are funded.
Changing providers carefully
If you are exploring another provider, tell us the support you want to replace or add and the timing you have in mind. You do not need to explain a private dispute to ask for an introduction. We can record preferences about contact and seek a new provider conversation, subject to capacity and permission.
Check your current service agreement for notice periods, cancellation charges and
arrangements for returning equipment or transferring necessary records. A new provider should confirm its start date and ability to deliver before you rely on the service. Capital Referral Network does not cancel your existing agreement by accepting a referral. Where essential supports may be interrupted, discuss continuity with the responsible professionals or organisations and agree who will manage the transition. We cannot guarantee replacement workers or immediate service commencement.
